Community Nurses

  • The nursing team provides high quality planned nursing care for individuals in their own homes or community setting.
  • Provides education, support and relevant information to help prevent health complications associated with immobility, disability or existing illness and adaptation to health related limitations.
  • Promotes and maintains independent living.
  • Promotes self-management and client involvement in planning and delivery of care.

Referrals to the district nursing service are usually reserved for individuals who are housebound – otherwise there should be a reason why a home visit is appropriate and this should be negotiated with the DN team.

Health Visitors

Our health visitor offers a health visiting service to all families with children under five years and can arrange to see you at home or in the surgery.  She is available to discuss any concerns parents may have about their child's growth, development or behaviour, as well as offering support to new parents and provide information on child/family friendly resources.
